This form should be completed by the faculty supervisor who wishes to hire a student for the Undergraduate Research Assistantship (URA) program. Students must have completed the URA Student Application before the supervisor completes this form; both student and supervisor will receive an email confirmation of the student's form submission.

Supervisor Contribution
If your student is eligible for the $400 School top-up (i.e. academic level of 3A or above, CAV is >= 80), the minimum supervisor contribution is $600 for the term.
If your student is not eligible, the minimum supervisor contribution is $1000.
President's Research Award
Effective Winter 2024: If your student is claiming a President's Research Award for working with you, they are not entitled to receive the $400 School top-up and a supervisor contribution is optional.
