This helps me to relieve stress and to focus energy on positive matters. Regardless you must be aware that I am under significant stress. > The University of Waterloo Disability Management Guide provides a summary of your sick leave and long-term disability benefits. It is available for you to access on the University of Waterloo website, on our department page Occupational Health. The link to the applicable page is below. > >   > > Absences and accommodations | Occupational Health | University of Waterloo (uwaterloo.ca) > >   > > On page 13 it states, “In most cases the Occupational Health Nurse will contact you if additional medical information is required, to determine if appropriate medical care is being received and to request updated medical documents as required. For prolonged absences monthly updates may be required.”. Above it says there that monthly updates May be required. You have a choice here.  Please note that I indicated in previous correspondence that contact with the workplace is stressful to me and precisely the reason for my sick leave right now.   > > As per the University of Waterloo policy, you have an obligation to provide medical information upon request and for prolonged absences every 4 weeks may be required. As your physician has not provided an end date for your medical leave, we are requesting that you provide updated medical information at an interval of every 4 – 6 weeks. > >   Where does it say must. Are you requesting or demanding? If you feel that monthly updates (or more information, or anything else) please write it to me and I will bring those questions to my Doctor. > I hope this helps to explain our process. I am not happy at all with your process. You as an RN should know employee health must come before strict adherence to "Policy". My Doctor has indicated that I am under workplace stress, yet these additional stresses are put on me. >   > > Sincerely, > > Karen > >     Richard Mann >   > > From: Richard Mann > Sent: Monday, November 22, 2021 7:25 PM > To: Karen Parkinson > Subject: Re: Sick leave Richard Mann > >   > > Dear Karen, > Can you please tell me where in that document it says I have an obligation to see treating physician every 4-6 weeks?  I did not see that.  Perhaps I missed it. > > Also, I have never seen this document before. > > > Thanks, >   Richard > > On 11/22/21 6:57 PM, Karen Parkinson wrote: > > Hello Richard, > > Thank you for your email explaining your current situation.   At this time, I understand and accept that you cannot commit to providing a specific date or time of when you will see your doctor next. > >   > > However, there is an obligation for you to participate in this sick leave process, this includes providing supporting documentation from your treating physician on a regular basis of every 4 – 6 weeks. I must advise you though that if you are unable to provide updated sick leave documentation in the above stated timeline your sick leave benefits will be at risk.
