Dear Dr. Mann,
I writing to advise that the 15 October and 20 October 2021 letters you received from the University of
Waterloo (University) regarding your non-compliance with the University of Waterloo’s COVID-19
Vaccination Requirement (the “Requirement”) were sent in error.
While you remain non-compliant with the Requirement and may be subject to disciplinary action in the
future for such non-compliance, the University acknowledges that all disciplinary action taken against
faculty must be in accordance with the Memorandum of Agreement between the University of Waterloo
and the Faculty Association of the University of Waterloo.
The University apologies for this error and I am writing to confirm the corrective action that the
University has taken to rectify this matter.
Our records indicate you have been on an approved medical leave since 10 November 2021.
The University retracts the two letters dated 15 & 21 October 2021 and I confirm that they have been
removed from your University employee file.
The University took steps to reimburse your pay from 20 October 2021 onwards in the gross amount of
$xxxx.xx. You received a net amount of $xxxx.xx via direct deposit to the account that is connected to
your payroll on Wednesday 17 November 2021.
The University sincerely apologizes for this administrative error.
